After processing this form, we will forward an official ADVISOR'S GUIDE containing further pertinent information. There is a one-time, non-refundable Registration Fee of $75 in addition to your above selected Per Person Package Plan (up to three (3) performing groups allowed per Registration Fee). This fee will be included on your invoice. Guides will not be sent until the registration fee is paid. Invoices will be emailed.
